Overview

Evergreen is seeking compassionate and talented people to join the team in its Skilled Nursing Facility!

Evergreen is a Christian, non-profit senior living organization serving older adults ages 55 and better. Located in a pleasant residential neighborhood in Oshkosh, Evergreen offers many options to meet unique needs.

We are home to almost 250 residents, and also a destination for families, community members, volunteers, non-profit groups and other visitors.

 

The available position is 2 shifts per two week pay period on the Day shift (7:30am - 4:00pm).  The shifts assigned are every other weekend.

Responsibilities

As a Skilled Nursing CNA Program Assistant you will:

 

  • Plan, develop, and implement an activity program for each resident based on individualized needs.
  • Plan unit activities
  • Develop and maintain an activities calendar
  • Provided stimulating opportunities for residents to maintain and enhance physical, social, and personal growth needs
  • Participate in weekly care planning and/or provide current information on residents being reviewed
  • Identify and write care plan approaches relative to program, restorative, and therapy, and follow through on implementation.
  • Complete assigned charting and documentation
  • Identify and make use of past leisure interests and hobbies of resident to stimulate active participation in activity/exercise programs
  • Make recommendations for use of adaptive equipment/devices to promote independence of residents
  • Transfer and ambulate residents
  • Assist with breakfast and dinner meals
  • Provide guidance and ideas to Nursing staff for weekend/evening activities
  • Ensure safety of residents at all times.
  • Encourage residents to be as independent as able in activities of daily living.
  • Build relationships with older adults in the place they call home.
  • Communicate effectively with coworkers, residents/clients and family members on a daily basis.
  • Promptly inform unit nurse of a change in resident’s condition and/or needs.

Qualifications

Minimum requirements:

  • At least 18 years of age
  • Nursing Assistant certification in the State of Wisconsin
  • “Can-do” attitude and a willingness to learn and take direction
  • Passion for working with older adults
  • Ability to communicate with all age groups
  • Strong prioritizing and organization skills
